The investigation began on Mar. 5, following allegations that vaccine doses were improperly administered at one of the pop-up vaccination clinics. It was conducted by public health and human resources.

The investigation found up to seven vaccine doses were given to individuals who “were related to one of the staff members whose actions were later investigated and/or who fell outside normal practice and the prescribed priority categories,” said a news release issued by the city.

The city says it has taken “appropriate” disciplinary action and one of the employees is no longer working with the city.

The names of the individuals and the location where the incident occurred are not being released.
