New program will help cut down on hospital readmissions

Getting out of the hospital after an illness can be a major relief, but sadly for many Canadians it’s short lived. One out of every 12 patients is readmitted within 30 days of discharge. After looking at the reasons, medical experts think they have the answers, and they’ve been tackling the issue at Hamilton General Hospital.
