Ontario’s information and privacy commissioner is investigating a privacy breach involving the personal health information of 6,500 Ontarians involved in a cancer care Ontario screening program.

There are reports the test results for colon cancer may have been lost in the mail.

Cancer Care Ontario says it’s also investigating whether fifteen reports containing patients’ personal information were successfully delivered to doctors back in February and March.

The agency used a Canada Post courier service to mail the reports which included the patients’ names, health card numbers and screening results.

The agency says Canada Post admitted that some of the reports were delivered without a signature confirmation.
